I need a registration form - 12-07-2008, 11:02 PM

ok i been trying to make a form so people can register for my site and i made one and for some reason it doesnt add the records to my db table. i notcied that the php script doesnt have an option for tbl_name.. it asks for username password db_name ect. but nothing for tbl_name
any suggestions?
