SQL database wont receive input

Hi,

It’s my first time using webhost. I’ve linked my html to the database I’ve created in Webhost, but it doesn’t seem to receive any input from anyone but me.
I’ve tried asking several people to reply to the form i’ve created, but I’ve only got one input on the database, even though more than 10 people have said they replied and got a “succesfull” message.

Any ideas why this might be happening??
