Trying to restore a forum config. I need database info

I had a phpbb3 forum on my site and I’m not sure what happened. An error occurred and all of the files from the FTP server were eliminated, but the database is still intact. I’ve already backed it up but I can’t just reinstall the files/forum. I have to follow steps from this guide:

I don’t know what any of this information is, or how to find it to fill out the notepad++ to create the config.php. How do I find this information?

I asked on the phpbb support boards and they told me to ask the host.

Can someone please help? I can supply the database for review.
